TSB Umpire Payroll Deduct Merchandise Order Form

This form to be used for orders requesting future payroll deduction. All items ordered with this form will be delivered in-person. No shipping option is available for items ordered using this form.

Items ordered with this form will be deducted from Umpire pay on the next pay date following the date of order (next pay date following the completion of an assignment).

Complete the Umpire Membership Agreement with your shirt and hat size. You will receive your shirt and hat prior to your first assigment (if the form has been submitted in time), and the $50 membership fee (WHICH INCLUDES A SHIRT AND HAT) will be deducted from your first payroll check/deposit. No money is due upfront.

To order additional items from the website (in addition to the hat and shirt that is included with your Umpire Membership Agreement), simply order and pay online, or if you would like to place an order and have the total deducted from your first check/deposit, use the Payroll Deduct Merchandise Order Form.

Items ordered through the Payroll Deduct Merchandise Order Form will not be received until you have arrived to work your assignment. A Director or UIC will deliver your items in-person. The items will then be deducted from your first payroll check/deposit.

Please note: We replenish our inventory on a weekly basis. Due to some items requiring custom embroidery, please allow approximately 10-12 days from the date of your order for shipment and/or pickup of certain items.

In stock items will be shipped within 2-3 business days.
